1、靜脈用藥調(diào)配使用操作規(guī)程一、靜脈用藥調(diào)配中心（室）工作流程臨床醫(yī)師開(kāi)具靜脈輸液治療處方或用藥醫(yī)囑→用藥醫(yī)囑信息傳遞→藥師審核→打印標(biāo)簽→貼簽擺藥→核對(duì)→混合調(diào)配→輸液成品核對(duì)→輸液成品包裝→分病區(qū)放置于密閉容器中、加鎖或封條→由工人送至病區(qū)→病區(qū)藥療護(hù)士開(kāi)鎖（或開(kāi)封）核對(duì)簽收→給患者用藥前護(hù)士應(yīng)當(dāng)再次與病歷用藥醫(yī)囑核對(duì)→給患者靜脈輸注用藥。二、臨床醫(yī)師開(kāi)具處方或用藥醫(yī)囑醫(yī)師依據(jù)對(duì)患者的診斷或治療需要，遵循安全、有效、經(jīng)濟(jì)的合理用藥原則，開(kāi)具處方或用藥醫(yī)囑，其信息應(yīng)當(dāng)完整、清晰。病區(qū)按規(guī)定時(shí)間將患者次日需要靜脈輸液的長(zhǎng)期醫(yī)囑傳送至靜脈用藥調(diào)配中心（室）。臨時(shí)靜脈用藥醫(yī)囑調(diào)配模式由各醫(yī)療機(jī)構(gòu)
2、按實(shí)際情況自行規(guī)定。三、審核處方或用藥醫(yī)囑操作規(guī)程負(fù)責(zé)處方或用藥醫(yī)囑審核的藥師逐一審核患者靜脈輸液處方或醫(yī)囑，確認(rèn)其正確性、合理性與完整性。主要包括以下內(nèi)容。（一）形式審查：處方或用藥醫(yī)囑內(nèi)容應(yīng)當(dāng)符合《處方管理辦法》、《病例書(shū)寫(xiě)基本規(guī)范》的有關(guān)規(guī)定，書(shū)寫(xiě)正確、完整、清晰，無(wú)遺漏信息。（二）分析鑒別臨床診斷與所選用藥品的相符性。（三）確認(rèn)遴選藥品品種、規(guī)格、給藥途徑、用法、用量的正確性與適宜性，防止重復(fù)給藥。（四）確認(rèn)靜脈藥物配伍的適宜性，分析藥物的相容性與穩(wěn)定性。（五）確認(rèn)選用溶媒的適宜性。（六）確認(rèn)靜脈用藥與包裝材料的適宜性。（七）確認(rèn)藥物皮試結(jié)果和藥物嚴(yán)重或者特殊不良反應(yīng)等重要信息。
