1、不同冠狀動脈病變程度患者血高敏C反應蛋白和尿酸的變化和意義張軍諸暨市第三醫(yī)院內(nèi)科311825摘要:目的:探討不同冠狀動脈病變程度的冠心病(CHD)患者血漿超敏C反應蛋白(hs-CRP)和尿酸(UA)水平相關(guān)性及其臨床意義。方法:經(jīng)冠狀動脈造影檢查者168例作為研究對象,根據(jù)冠狀動脈病變指數(shù),分為單支病變組40例,雙支病變者47例,多支病變組45例和冠狀動脈造影正常的對照組36例。測定血漿hs-CRP、UA、TC、TG、HDL-C、LDL-C。計算體重指數(shù)(BMI)、腰臀比(WHR)和Gensini積分。結(jié)果:CHD各組血漿hs-CRP、血清中UA水平明顯高于對照
2、組(P<0.01),血漿中hs-CRP水平與UA、WC、BMI、WHR、TC、LDL-C、Gensini積分呈顯著負相關(guān)(P<0.05或P<0.01),均衡年齡、血脂、肥胖等因素影響后,hs-CRP和UA與Gensini積分存在相關(guān)性。結(jié)論:冠心病患者血漿CRP、UA水平明顯增高,并與冠狀動脈病變程度密切相關(guān),可以反應心肌受損的程度,具有重要的臨床意義。關(guān)鍵詞:C反應蛋白;尿酸;冠狀動脈疾病CorrelationofUricAcidandHigh-SensitivityC-ReactiveProteinwithSeverityofCoronaryArteryDi

5、ofhs-CRP,UA,totalcholesterol(TC),triglyceride(TG),high-densitylipoproteincholesterol(HDL-C),low-densitylipoproteincholesterol(LDL-C),apoprotein-A1(apo-A1),andcalculatingbodymassindex(BMI),waist-hipratio(WHR)andGensiniscoreofcoronarystenosis.ResultTheconcentrationofhs-CRPandUAinCHDgro
6、upsweresignificantlyhigherthanthoseinthecontrolgroup(P<0.01).Theplasmahs-CRPlevelswerenegativelyandsignificantlycorrelatedwithUA,WC,BMI,WHR,TC,LDL-C,andGensiniscore(P<0.05or0.01).Balancedtheinfluentialfactors(suchasage,bloodglucose,bloodlipidsandobesity),Hs-CRPandUAstillhadsignifican
7、tlycorrelatedwithGensiniSscore.Conclusion:Theincreasedplasmahs-CRPandUAhadcloserelationshipwiththeseverityofcoronaryarterystenosisinpatientswithCHD.Bothofthemareimportantclinicalparameterstoreflectseverityofmyocardialnecrosisandhaveimportantclinicalpractice.KeywordsHigh-sensitivityC-
