1、東南大學碩士學位論文冠狀動脈瘺外科治療的臨床研究姓名：盧子潤申請學位級別：碩士專業(yè)：臨床醫(yī)學（本碩連讀）指導教師：陳鑫2012-05-31中義摘要冠狀動脈瘺外科治療的臨床研究中文摘要研究生盧子潤導師陳鑫教授東南大學醫(yī)學院目的總結(jié)外科手術(shù)治療冠狀動脈瘺的臨床經(jīng)驗及治療結(jié)果。方法回顧性分析2005年2月至2011年6月12例冠狀動脈瘺患者的臨床資料，其中男6例，女6例；年齡4"--77(50．9123．8)歲。合并冠狀動脈瘤樣擴張5例，合并卵圓孔未閉1例，合并冠狀動脈粥樣硬化性心臟病2例，其中1例同時合并重度主動脈瓣狹窄。l例為非體外循環(huán)心臟不停跳下手術(shù)，余1l例在體外循環(huán)下行手術(shù)治療，其
2、中3例心臟不停跳。l例同期行主動脈瓣置換術(shù)+冠狀動脈旁路移植術(shù)。結(jié)果所有患者康復出院。手術(shù)時間90---,245(151．25--+42．65)分鐘，體外循環(huán)時問3l～116(65．06±29．16)分鐘，主動脈阻斷時問18"--97(43．00±33．41)分鐘；術(shù)后住院時問10一--．15(12．5±1．45)天。全組住院期問無圍術(shù)期死亡，自覺癥狀消失。無心肌缺血及殘余瘺等并發(fā)癥。術(shù)后心功能NYHA分級改善至I、II級。術(shù)后門診及電話隨訪，隨訪率100％，隨訪時間4個月"---6年，隨訪終止時無再次手術(shù)病例，無死亡病例。結(jié)論冠狀動脈瘺一旦明確診斷則均應盡早手術(shù)治療。手術(shù)效果十分滿意
