1、萬(wàn)方數(shù)據(jù)鼻咽癌三種布野方案對(duì)調(diào)強(qiáng)放療劑量分布的影響伍志紅張九堂’曾彪魯旭蔚(湖南省腫瘤醫(yī)院放療綜合科，湖南長(zhǎng)沙410013)[摘要]【目的】比較鼻咽癌調(diào)強(qiáng)放療三種布野方案之間的差異，為鼻咽癌調(diào)強(qiáng)計(jì)劃選擇布野方案提供參考。【方法】對(duì)lo例鼻咽癌病例選用A、B、C三種布野方案，應(yīng)用相同的物理目標(biāo)函數(shù)和約束條件，利用直接子野優(yōu)化進(jìn)行鼻咽癌調(diào)強(qiáng)放療計(jì)劃的設(shè)計(jì)。用劑量體積直方圖(DVH)，靶區(qū)適形度指數(shù)(cI)，危及器官劑量分布，機(jī)器跳數(shù)，子野數(shù)目以及治療過(guò)程中是否需要2次擺位作為計(jì)劃評(píng)價(jià)指標(biāo)進(jìn)行比較。【結(jié)果】A，B，C三種方案中，
2、脊髓，腦干，垂體．晶體，口腔的體積量沒有明顯的區(qū)別，A，B兩種7野方案，靶區(qū)覆蓋度(1395，V100)，靶區(qū)均勻性(Dl一／鞏。)。適形度指數(shù)(CI)，均無(wú)顯著性差異(P>O．05)。但是A方案治療過(guò)程中需要2次擺位。9野方案即C方案的靶區(qū)覆蓋度，靶區(qū)均勻性比A，B方案稍好，但包括靶區(qū)適形度無(wú)顯著性差異(P>o．05)，但機(jī)器跳數(shù)和子野教目均比A，B方案低5％，治療過(guò)程中也需要2次擺位?！窘Y(jié)論】利用直接子野優(yōu)化進(jìn)行鼻咽癌調(diào)強(qiáng)放療計(jì)劃的設(shè)計(jì)，7野方案就足以滿足臨床的要求，考慮到治療過(guò)程中擺位的重復(fù)性以及放射生物效應(yīng)，B方案
