1、胃腸病學(xué)2008年第13卷第5期腸內(nèi)生態(tài)免疫營養(yǎng)對豬重癥急性胰腺炎模型免疫功能的影響田覓1鄒曉平,宰陳磊2王莉,顧超2南京醫(yī)科大學(xué)鼓樓臨床醫(yī)學(xué)院消化內(nèi)科1(210008)南京大學(xué)醫(yī)學(xué)院附屬鼓樓醫(yī)院消化內(nèi)科2背景:重癥急性胰腺炎(SAP)患者存在不同程度的免疫功能抑制,與疾病的發(fā)展和轉(zhuǎn)歸密切相關(guān)。研究顯示腸內(nèi)營養(yǎng)(EN)尤其是生態(tài)免疫營養(yǎng)(EIN)應(yīng)用于SAP較腸外營養(yǎng)(PN)有顯著優(yōu)越性。目的:探討腸內(nèi)EIN對豬SAP模型免疫功能的影響。方法:制備豬SAP模型,18只實(shí)驗(yàn)豬隨機(jī)分為PN組、腸內(nèi)要素營養(yǎng)(E
2、EN)組和EIN組,分別予營養(yǎng)支持8d。動態(tài)檢測各組血清、胰淀粉酶水平、T細(xì)胞亞群和血清IgG、IgA、IgM水平,觀察胰腺組織病理學(xué)改變。結(jié)果:造模后各組血清、胰淀粉酶水平明顯升高,CD3+、CD4+T細(xì)胞和CD47CD8+比值降低,血清IgG、IgA、IgM水平降低.予營養(yǎng)支持后上述指標(biāo)均有所改善(除PN組血清IgG水平)。除血清淀粉酶和IgM水平,EIN對其余各指標(biāo)的改善作用均較PN和EEN更為明顯(P<0.05)。三組間胰腺組織病理學(xué)評分無明顯差異。結(jié)論:早期腸內(nèi)EIN增強(qiáng)豬SAP模型免疫功能的作
