1、一中文摘要研究背景:目前國(guó)內(nèi)外關(guān)于顯微淋巴移植治療乳腺癌術(shù)后上肢淋巴水腫最新的研究進(jìn)展,提出了兩個(gè)觀點(diǎn):一是通過(guò)顯微手術(shù)操作將取自股區(qū)的淋巴結(jié)移植至腋區(qū)H鍆,二是通過(guò)顯微手術(shù)操作將取自股區(qū)的淋巴結(jié)移植至腕部H61。雙方都各自提出了顯著的療效。本研究首先將現(xiàn)有的制作犬后肢淋巴水腫模型的方法進(jìn)行改進(jìn)(預(yù)試驗(yàn)),然后通過(guò)對(duì)犬淋巴水腫模型進(jìn)行手術(shù)治療,運(yùn)用顯微技術(shù)操作將取自股區(qū)的淋巴結(jié)移植至腋區(qū)或腕部。最后通過(guò)評(píng)估比較其療效,從而選擇最佳的手術(shù)方式并應(yīng)用于臨床患者。研究目的:模擬臨床乳腺癌根治術(shù)后放療患者的淋巴水腫形成過(guò)程,建立并觀察一種改良的犬肢體淋巴水腫動(dòng)
2、物模型,為今后的實(shí)驗(yàn)和臨床研究作基礎(chǔ)。研究方法:應(yīng)用淋巴結(jié)清除術(shù)與術(shù)前及術(shù)后各一次X—ray照射相結(jié)合,制作犬后肢淋巴水腫模型。術(shù)后觀察肢體周徑、體積及形態(tài)變化,并行淋巴核素造影。研究結(jié)果:動(dòng)物死亡率為0%,動(dòng)物模型成功率達(dá)100%,犬后肢在術(shù)后16周左右產(chǎn)生相應(yīng)的形態(tài)變化及淋巴造影表現(xiàn),其水腫形成規(guī)律與臨床患者相似。研究結(jié)論:該模型成功率高,并縮短了形成肢體慢性淋巴水腫的時(shí)間。而且犬后肢淋巴解剖與人較接近,便于觀察;由于血管和淋巴管口徑較粗,尤其適用于顯微淋巴外科的應(yīng)用及研究?!娟P(guān)鍵詞】淋巴水腫,動(dòng)物模型,淋巴結(jié)移植Basicresearchofad
