1、小兒惡性腫瘤化療前后外周血多藥耐藥基因、多藥耐藥相關(guān)蛋白基因、肺耐藥蛋白基因的變化及臨床意義鄧曉斌,王珊,鄒琳,李長春,章均,歐陽軍(400014重慶,重慶醫(yī)科大學(xué)附屬兒童醫(yī)院腫瘤外科)[摘要]目的探討實體惡性腫瘤化療患兒外周血單個核細胞(peripheralbloodmononuclearcells,PBMCs)中多藥耐藥基因(multi-drugresistance1,MDR1)、多藥耐藥相關(guān)蛋白基因(multi-drugresistance-associatedprotein1,MRP1)、肺耐藥蛋白基因(lungresistanceprotein,LRP)的表達量及其與
2、臨床化療的關(guān)系。方法應(yīng)用實時熒光定量PCR(real-timefluorescencequantitativePCR,F(xiàn)Q-PCR)技術(shù),檢測患兒化療前后外周血和手術(shù)切除新鮮凍存組織,以及對照組外周血中MDR1、MRP1、LRP的mRNA表達量。結(jié)果所有化療患兒外周血MDR1表達量與同期手術(shù)切除的腫瘤組織MDR1表達量存在正相關(guān)(r=0.894,P=0.000);化療前后外周血MDR1表達量變化與腫瘤大小變化間存在負相關(guān),神經(jīng)母細胞瘤(r=-0.733,P=0.000),卵黃囊瘤(r=-0.874,P=0.000),淋巴瘤(r=-0.617,P=0.005);化療前外周血MDR
3、1表達量與首次化療后相應(yīng)腫瘤標(biāo)志物變化大小呈負相關(guān),神經(jīng)母細胞瘤尿香草杏仁酸(VMA)(r=-0.440,P=0.046),卵黃囊瘤甲胎蛋白(AFP)(r=-0.831,P=0.000),淋巴瘤乳酸脫氫酶(LDH)(r=-0.495,P=0.031)。隨著化療次數(shù)的增加MDR1表達量增加(P<0.05)。結(jié)論動態(tài)檢測患兒化療前后PBMCs中MDR1mRNA水平對化療療效及預(yù)后的判定有一定的指導(dǎo)意義,可在一定程度上指導(dǎo)化療方案的修訂。[關(guān)鍵詞]小兒實體惡性腫瘤;多藥耐藥基因;多藥耐藥相關(guān)蛋白基因;肺耐藥蛋白基因;外周血;熒光定量PCR[中圖法分類號][文獻標(biāo)志碼]A[基金項目]重
