1、萬方數(shù)據(jù)肝臟局灶結(jié)節(jié)性增生238例臨床病理分析陳伶俐紀元許建芳盧韶華侯英勇侯君宿杰·阿克蘇曾海英譚云山【摘要l目的分析肝臟局灶結(jié)節(jié)性增生(FNH)的臨床病理特點。方法回顧性分析238例FNH患者的臨床及影像資料，復(fù)習(xí)大體標本及光鏡下組織學(xué)特點，按照現(xiàn)有的分類標準對FNH行組織學(xué)分類，并對所有患者隨訪。結(jié)果238例FNH患者中男性145例，女性93例，年齡11—77歲，平均年齡39．1歲。有完整I臨床資料的233例中，188例無相關(guān)癥狀，216例無乙型和(或)丙型病毒性肝炎史，232例患者血清甲胎蛋白為陰性。有影像結(jié)果的185例中123例(66．5％)經(jīng)影像診斷為FNH。大體觀察上238例FN

2、H標本包含284枚病灶，平均直徑3．7cm，215例(90．3％)單發(fā)，172枚發(fā)生于肝右葉，115枚(柏．5％)切面可見星狀瘢痕或呈結(jié)節(jié)狀。光鏡下觀察238枚FNH病灶，229枚(96．2％)為經(jīng)典型FNH，6枚為毛細血管擴張型FNH，3枚為混合性增生與腺瘤樣型FNH，未見到細胞不典型FTqH。173例患者獲得隨訪結(jié)果，無FNH相關(guān)性死亡，2例患者于術(shù)后2年和4年復(fù)發(fā)。結(jié)論FNH是對肝臟內(nèi)局部異常血流的反應(yīng)性肝細胞增生性疾病，無女性好發(fā)傾向；毛細血管擴張型FNH應(yīng)歸類為肝細胞腺瘤，該組無細胞不典型FNH；超過66％的病例可經(jīng)影像診斷。I關(guān)鍵詞】肝臟疾?。辉錾?；診斷Focalnodularh
