1、中國熱帶醫(yī)學(xué)2017年第17卷第4期ChinaTropicalMedicine,April2017,Vol.17,No.4·411··短篇論著·早中孕期胎兒畸形超聲篩查方法分析徐世亮,寶波,李娜,魏珊,楊暖中南大學(xué)湘雅醫(yī)學(xué)院附屬??卺t(yī)院,海南???70208摘要:目的探討二維超聲和二維聯(lián)合實(shí)時(shí)三維超聲在孕早中期篩查胎兒畸形的價(jià)值,為臨床推廣無創(chuàng)傷篩查方法提供基礎(chǔ)資料依據(jù)。方法回顧性分析中南大學(xué)湘雅醫(yī)學(xué)院附屬??卺t(yī)院于2013年1月—2015年6月共4929例產(chǎn)婦經(jīng)產(chǎn)前二維超聲和二維聯(lián)合實(shí)時(shí)三維超聲對(duì)比檢查結(jié)果,并對(duì)比分析追蹤引產(chǎn)或分娩的結(jié)
2、果。結(jié)果引產(chǎn)或分娩后證實(shí)胎兒畸形151例,二維超聲檢出的胎兒畸形108例,準(zhǔn)確率為71.5%,二維超聲聯(lián)合實(shí)時(shí)三維超聲檢出胎兒畸形135例,準(zhǔn)確率為89.4%,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。二維超聲聯(lián)合實(shí)時(shí)三維超聲在胎兒神經(jīng)系統(tǒng)畸形、先天性心臟畸形、消化道畸形、唇腭裂、兩種及以上多種畸形等的檢出準(zhǔn)確率要明顯高于單獨(dú)使用二維超聲,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論二維超聲聯(lián)合實(shí)時(shí)三維超聲檢查可以明顯提高胎兒畸形的篩查率,有助于優(yōu)生優(yōu)育。關(guān)鍵詞:二維超聲;實(shí)時(shí)三維超聲;早中孕期;胎兒畸形中圖分類號(hào):R714.43文獻(xiàn)標(biāo)識(shí)碼:B文章編號(hào)

6、wereconfirmed151cases,108fetalmalformationswerefoundbysingletwo-dimensionalultrasound,butafterinducedabortionanddelivery,thediagnosticaccuracywas71.5%,while135fetalmalformationswerefoundbycombineddetection,thediagnosticaccuracywas89.4%.Thedifferencewasstatisticallysignifi
7、cant(P<0.05).Thediagnosticaccuracyofthefetalmalformation,suchasneuralmalformation,congenitalheartmalformation,digestivetractmalformation,cleftlipandpalate,byusingcombineddetectionwassignificantlyhigherthansingletwo-dimensionalultrasound.Thedifferencewasstatisticallysignif
8、icant(P<0.05).ConclusionTwo-dimensionalultrasoundcombinedwiththree-dimensionalultrasoundcanmarke
