1、液基細胞學(xué)檢查聯(lián)合高危型HPV檢測在宮頸癌篩查中臨床應(yīng)用doi:10.3969/j.issn.1007-614x.2014.12.74摘要目的:探討液基細胞學(xué)(TCT)檢查聯(lián)合高危型人乳頭瘤病毒(HC-IIHPV-DNA)檢測在宮頸癌篩查中的臨床意義。方法:2012年1月-2013年8月收集自愿接受宮頸癌篩查的婦女568例,行TCT檢查,其中66例結(jié)果為不典型鱗形細胞(ASCUS)及以上,患者同時行高危型HPV檢測和陰道鏡檢查并活檢,分析兩種方法對診斷宮頸病變的臨床意義。結(jié)果:TCT檢查結(jié)果異常ASCUS30
2、例,低度鱗狀上皮內(nèi)瘤變(LSIL)21例,高度鱗狀上皮內(nèi)瘤變(HSIL)13例,鱗狀上皮細胞癌(SSC)2例。細胞學(xué)與病理診斷符合率為LSIL73.3%(22/30),HSIL92.3%(12/13),SCC100%(2/2)。高危型HPV感染率51.51%(34/66),高危型HPV在不同宮頸病變中的陽性率為ASC-US26.66%(8/30),LSIL66.66%(14/21),HSIL76.92%(10/13),SCC100.00%(2/2)o高危型HPV陽性率隨宮頸病變程度加重而明顯升高。TCT檢查的
3、敏感性81.53%,特異性91.73%;HC-IIHPV-DNA檢測的敏感性92.43%,特異性95.69%o結(jié)論:TCT結(jié)合HC-IIHPV-DNA檢測增加了宮頸病變診斷的敏感性和特異性,提高了宮頸癌篩查的準(zhǔn)確性。關(guān)鍵詞TCT高危型HPV宮頸癌篩查ClinicalapplicationofliquidbasedcytologycombinedwithhighriskHPVtestingincervicalcancerscreeningHanYanmin,DengYaTheTenthPeople,sHospi
