5、G)圖形評(píng)分上的對(duì)比分析,探討aEEG評(píng)分系統(tǒng)評(píng)估早產(chǎn)兒腦成熟度的臨床價(jià)值?!痉椒ā抗布{入32~36周的早產(chǎn)兒114例,其中SGA組54例,AGA組60例,記錄入選早產(chǎn)兒的一般情況,并采用NicoletOne腦功能監(jiān)護(hù)儀,均于生后3天內(nèi)進(jìn)行監(jiān)測,每次連續(xù)監(jiān)測至少3小時(shí),排除阻抗大于20Q的記錄。運(yùn)用早產(chǎn)兒aEEG評(píng)分系統(tǒng),計(jì)算兩組早產(chǎn)兒aEEG圖形的連續(xù)性、睡眠一覺醒周期、下邊界振幅、帶寬和總評(píng)分各參數(shù)的分值,分析兩組早產(chǎn)兒不同出生胎齡下其aEEG圖形評(píng)分的特點(diǎn),以及在相同出生胎齡下兩者在aEEG圖形評(píng)分上的差異?!窘Y(jié)果】1.SGA組與aEEG評(píng)分:隨著出生胎
6、齡的增加,aEEG圖形連續(xù)性、睡眠.覺醒周期、下邊界振幅、帶寬和總評(píng)分分值均逐漸增加,不同胎齡組間的差異有統(tǒng)計(jì)學(xué)意義(分別為F=4.145,P=0.006;F=14.368,P=0.000;F=2.624,P=0.046;F=7.203,P=0.000;F=9.271,P=0.000)。2.AGA組與aEEG評(píng)分:隨著出生胎齡的增加,aEEG圖形睡眠.覺醒周期、帶寬和總評(píng)分逐漸增加,不同胎齡組間的差異有統(tǒng)計(jì)學(xué)意義(分別為F=6.122,P=0.000;F=9.121,P=0.000;F=10.464,P=0.000),而連續(xù)性、下邊界振幅不同胎齡組間的差異無統(tǒng)
7、計(jì)學(xué)意義(分別為F=2.200,P=0.081;F=0.587,P=0.673)。3.a(chǎn)EEG評(píng)分與胎齡的相關(guān)性分析:SGA組連續(xù)性、睡眠.覺醒周期、下邊界振幅、帶寬和總評(píng)分均與出生胎齡明顯相關(guān),相關(guān)系數(shù)分別為0.438、0.597、0.385、0.606、0.608,P均<0.05;AGA組的睡眠.覺醒周期、帶寬和總評(píng)分與出生胎齡有相關(guān)關(guān)系,相關(guān)系數(shù)分別為0.528、0.615、0.635,P均<0.05。萬方數(shù)據(jù)4.相同出生胎齡下,SGA組在帶寬評(píng)分和總評(píng)分上落后于AGA組,而在連續(xù)性、睡眠.覺醒周期、下邊界振幅的分值上,差異無統(tǒng)計(jì)學(xué)意義?!窘Y(jié)論】1.早產(chǎn)
