1、腦發(fā)育性靜脈異常的CT、MRI診斷作者:馬永華,許守利,孫建剛,劉華,張榮坤單位:山東省泗水縣人民醫(yī)院影像科【摘要】目的探討腦發(fā)育性靜脈異常的CT、MR1表現(xiàn),提高影像學(xué)診斷水平。方法搜集經(jīng)病理及DSA證實(shí)的DVA38例,7例手術(shù)病理證實(shí),31例DSA證實(shí),全部病例均行MRI平掃及增強(qiáng)掃描,8例行CT檢查,冋顧性分析其CT、MRI表現(xiàn)。結(jié)果發(fā)生于幕下11例,幕上27例。8例行CT檢查中,4例平掃未見明顯異常密度灶,2例可見右側(cè)小腦半球出血灶,1例右側(cè)額葉見高低密度混雜病灶。1例增強(qiáng)掃描見右側(cè)額葉條紋狀異常增強(qiáng)血管影及粗大的引流血
2、管相連。MR掃描顯示淸晰,TlWkT2WI表現(xiàn)為條狀血管流空影32例,并發(fā)海綿狀血管瘤5例,表現(xiàn)為不均勻短T1長(zhǎng)T2異常混雜信號(hào)。增強(qiáng)掃描顯示明顯強(qiáng)化的髓靜脈及引流靜脈,呈典型“水母頭”狀。幕卜DVA引流靜脈匯入腦表而靜脈者4例,匯入第四腦室的室管膜下靜脈7例,幕上DVA引流靜脈經(jīng)皮層靜脈匯入腦衣血靜脈竇者15例,匯入室管膜下靜脈者12例,即匯入腦表面靜脈乂匯入腦深靜脈者1例。結(jié)論CT診斷對(duì)該病特異性較小,MRI增強(qiáng)掃描多方位觀察,可清楚顯示DVA的典型表現(xiàn),MR檢查可完全替代DSA成為DVA首選及確診檢
