1、雙鋼板治療成人肱骨遠端復雜粉碎骨折療效分析【摘要】[目的]討論成人肱骨遠端復雜粉碎骨折使用雙鋼板內固定的手術方法及療效。[方法]回顧分析本院2004年4月-2007年4月應用雙鋼板內固定治療成人肱骨遠端復雜粉碎性骨折28例，骨折按AO/ASIF分型法分型，均為C型，其中C1型9例，C2型15例，C3型4例。均取肘后正中切口，行切開復位雙鋼板螺釘內固定治療。其中經肱三頭肌舌形瓣15例，尺骨鷹嘴截骨13例(包括伴同側鷹嘴骨折1例)。手術先行關節(jié)內骨折關節(jié)面復位，細克氏針臨時固定，使復雜粉碎骨折變?yōu)楹唵蔚碾殴趋辽瞎钦郏缓笤傩懈慎慷藦臀?。內固定方式，按?/p>

2、折情況選用型號大小相吻合的重建鋼板固定肱骨遠端橈側柱后方，以塑形良好的1/3管型鋼板固定肱骨尺側柱的內緣，這樣就在二個互成90°的平面上分別固定了肱骨髁的內外側柱。[結果]本組隨訪8～33個月，平均隨訪17個月，骨折愈合時間8～16周，23例術后功能恢復良好，有1例骨化性肌炎形成，4例關節(jié)間隙狹窄，無骨不愈合及尺神經損傷病例，優(yōu)良率82.1%。[結論]成人肱骨遠端復雜粉碎骨折，結構復雜，手術難度大，切開復位雙鋼板螺絲釘內固定治療，固定強度和抗疲勞作用均強于其他固定方式，有利于肘關節(jié)的早期活動和防止粘連，能有效地恢復肘關節(jié)功能，是一種較理想的治療方法
