6、········································41中文摘要新型髖臼后壁可塑形解剖板治療髖臼后壁骨折合并髖臼唇損傷的生物力學(xué)研究摘要目的:由于交通事故與工傷事故的日漸增多,髖臼骨折的發(fā)生率也逐步增多,高能量創(chuàng)傷常引發(fā)髖臼骨折。髖臼后壁骨折是髖臼骨折最常見的一種,大約占全部髖臼骨折的1/4~1/3。如果在發(fā)生事故時(shí),髖關(guān)節(jié)處在屈曲、內(nèi)收、內(nèi)旋位,極大的高能量暴力通過膝關(guān)節(jié)傳導(dǎo)極易造成髖關(guān)節(jié)脫位,如果下肢的內(nèi)收角度偏小,會(huì)造成股骨頭撞擊髖臼后緣,極易發(fā)生髖臼后唇撕裂骨折或股骨頸骨折等并發(fā)癥。現(xiàn)在治療髖臼后壁骨折的內(nèi)固定
7、物較多,而且取得了較好的結(jié)果,但研究合并髖臼唇損傷的修復(fù)研究尚且較少,尤其是內(nèi)固定修復(fù)治療髖臼后壁骨折合并髖臼后唇撕裂損傷未見相關(guān)資料報(bào)告。本實(shí)驗(yàn)通過生物力學(xué)比較與研究,對馮文嶺教授設(shè)計(jì)的新型髖臼后壁可塑形解剖板治療髖臼后壁骨折合并髖臼唇損傷提供試驗(yàn)依據(jù)。該新型髖臼后壁可塑形解剖板用于解決在髖臼骨折接骨時(shí)使用多塊接骨板時(shí)結(jié)構(gòu)繁雜、連接不穩(wěn)固、應(yīng)用不方便、不能修復(fù)髖臼唇的缺點(diǎn),其設(shè)計(jì)原理是:它為薄金屬板體,板的主體為長條形接骨板,長條接骨板的兩側(cè)邊向外延伸成側(cè)孔,其靠近髖臼唇一側(cè)可以通過可吸收縫合線修復(fù)髖臼盂唇,另外一側(cè)可以擰入螺釘,加強(qiáng)解剖板的固
8、定強(qiáng)度。該弧形板面外形與坐骨結(jié)節(jié)外形相匹配,彎曲弧度與髖臼外面弧形外形相匹配,可能成為治療髖臼后壁骨折合并髖臼唇損傷更完美的內(nèi)固定材料。
